We will provide the exam forms for you to submit, and the exam results will be electronically submitted to FMCSA on the same day.
When you come in:
  1. Bring your current license and medical card.
  2. Fill out Form MCSA-5875 (pages 1 and 2) ahead of time to speed up your exam. 
  3. Bring comprehensive medical records from your primary care doctor if your primary care doctor is not the one performing your exam.
  4. If you have any health conditions, bring the name and contact information for the doctor who treats you for each condition. Conditions like high blood pressure, diabetes, or sleep apnea typically require additional paperwork, such as sleep test results or stress test results.
  5. Bring a complete medication list. The list should include every medication you take, its strength, its dosage, and the prescribing doctor.
  6. Avoid coffee or energy drinks the day of your exam as it increases blood pressure.
  7. A urine collection is required so avoid urination immediately before your appointment.
  8. If you wear glasses or a hearing aid to drive, please bring them with you. Please note, if you wear contact lenses, you will be asked to remove them for a portion of the eye exam.
